By limiting data collection, requiring consent, and using secure systems, we uphold the principles of dignity, choice, control, and transparency in the Aged Care Quality Standards and NDIS Code of Conduct.
To protect privacy, please avoid uploading diagnoses, clinical notes, or sensitive documents. If more information is needed, we will contact you to arrange a secure transfer.
This form complies with the Australian Privacy Act and relevant Aged Care and NDIS regulatory requirements.
Information is used solely to contact the consumer and provide the support you have requested.
We do not share information with any third party without the consumer’s consent.
You should only complete this referral if the consumer has agreed to it.
